First Phase~
Medical Health Exam 

1. Review of your past medical health history and vaccination records. 
2. Prescribe vaccines as needed
3. Order Lab tests as needed. 

Second Phase~

• Review of all tests and vaccinations done
• Completion of forms 
• Final signatures. 
• Handing over the signed i693 in a sealed envelope.
